It happened in one of the World Cups in the 70's.

It was H.Hunt in the 1975 World Cup Championship match between England and France.
Referee Keith Page was taken off the field after 27 minutes. He had suffered a severe reaction to some inoculation injections he'd received just a day before the match. H.Hunt, the touch judge was called on to referee the rest of the match and a sideline official from an earlier match was brought on to complete the game as the new official.

Was it before mandatory finals were introduced?
We must remember that mandatory finals have been in place twice before.
However if you are referring solely to mandatory grand finals, than the answer is yes.
