Dunno about the others but in the Titans case I'd say its because they sacked the bloke who was having a go, and didn't sack those who weren't. I'd include Mal in that.
If they utilise and nurture their juniors, and go on a fan drive to stir up support and get ppl to the game, they will eventually get there. They simply need the right people guiding this club.

I though the salary cap leads to a fairer competition apparently not
But their culture is now great with Mal the immortal.
Until the rules are changed to allow the smaller skilled players back in the game, the very few talented players will always be signed to just a hand full of clubs.
