Bad rugby league decisions

What stand out as some of the worst rugby league decisions made by clubs in recent times?

Ivan Cleary not being offered an extension (or more than a season as it may've been) by the Warriors in 2011 certainly looks like a terrible call. If ever there was a coach who seemed to understand the club and get the best out of them it was him, and he left when they should've been on the verge of good times. Instead they've pretty much squandered one of the more talented rosters they've had through now four coaches.

Getting rid of Ennis for Lichaa could've been great but that's definitely a hindsight one that looks horrendous.

I think another one was probably Newcastle pissing Brian Smith off. That was a side that looked like being on the up but when they decided to let him go they fell apart again. Bar the brief period after with Bennett they've looked dreadful ever since (may be on the up now, but up is the only direction from where they've been).
