The main thing is volume; she latches well, wife has good technique etc. There's just not enough sadly It's coming though.
Ok, so here goes some helpful ideas.
First the general principles.
Breast milk is made on the principle of supply and demand
a feed consists of two parts (like meals for us if you like ) the fore milk is the entree and the hind milk is the main course.If a baby gets more of the watery fore milk and then comes off ,and then feeds again in say 20 minutes because they have only had mostly watery milk then they again get the fore milk ...and so on. that's why it's important that bub gets to the hind milk at every feed .You can tel that the hind milk has come because bub will gulp it down( because it comes out faster)
When bub gets more of the hind milk they will be more satisfied,gain weight ,and mum will make more milk.
The more bub takes of the hind milk ,the more mum will make.
The best things for a blocked duct are more properly positioned feeding ,a warm shower and a gentle rub of the area( the beer idea is really an old wives tale )

Nooo the nipples don't have to toughen p .Bub needs to be attached properly and mum should be relaxed .
When a baby is feeding properly the nipple goes down the back to the throat and is not rubbing against anything.The baby's mouth is wrapped around the areole area( the pumping spot for the milk ducts if you like) They used to talk about the Special K mouth.
But you are on to something with the direct feeding.Best solution for all the issues of supply .

The beer thing worked a treat for the wife, and we used a warm compress instead of a shower. Massage was a tough thing to do since she was so engorged and uncomfortable so the compress helped a fair bit

"Toughen up" was a bad choice of words - conditioned is a better one - and that refers more to the suckling technique of the child than the nipples themselves. It can take a few weeks for the baby to figure out how to feed more effectively. The Wife also used a 'boppy' U shaped pillow to support the baby at an easier height so she could focus more on positioning instead of support. Also, have a glass of warm water or juice on hand for her Drew!

On the beer thing though,if you think its working thats fine,but you might want to think about beer directly after a feed rather than a few hours after.That way the smalcohol is out of her system when baby comes to feed.
