Look in the Customise Game->Customise Team menu. There are some detailed instructions in the manual to take it from there.

Mate we make files for the PC version by swapping the inbuilt files with ones we made ourselves, you cant do that on PS2 unfortunately. You will just have to settle for the custom jerseys that come with the game
