Last one i can recall was 1993 when Polla Mounter fouled a bears player as he put the ball down.
The bears were down 17-0 and won 18-17 thanks to the 8 point try.

There is a penalty try - where a try is awarded when no try was actually scored - but as beekershark pointed out - a try would for certain have been scored if it wasnt for the infringement.
And there are penalties awarded for infringements after the try is scored.
In both instances a regular 4 point try is awarded - but the goal kicker is awarded two shots at goal - one from where the try is scored (as usual) and one from directly in front.
So there is an opportunity to recieve 8 points - but the try itself is only worth the regular 4 points.

for an 8 pointer to be awarded, the try has to be scored and foul play made afterwards or during the scoring of that try, Ie the try scorer puts the ball down, but if his support player is taken high and cheap off the ball then this can be an 8 pt try.

as someone else mentioned, the only difference to a normal try, is you get a second penalty shot from right in front, so you are pretty much guaranteed to get at least 6 points, from the infringement, if the other shot is out wide

Now the foul has to be at the exact same time. So if he scores a try, then a player comes through with his knees or late tackle, then it's no longer an 8-point try. It has to be at the same instant.
