Queensland could of honestly been penalised a lot more.
The first 4 or so penalties came quick including a couple of very soft penalties. From there smith tried to call the refs bluff. He thought that the ref wouldn't possibly keep penalizing queensland (refs usually try go for an even penalty count) so queensland were slowing down the ruck to a stand still, off side etc thinking there ref won't keep blowing the whistle. But the ref called his bluff and kept penalizing the slow play the balls and offsides. Only after it got to 9-1 did the refs shit themselves and let Queensland get away with all but that most blatant penalties
