They are examining whatever factors contributed to the death. Doesn't necessarily matter if that behaviour occurs frequently. It's similar to an inquiry that finds that speeding contributed to a fatal accident. It's a coronial inquiry.

I understand mate. But the Coroner's job is to find out what happened, and what contributed to that happening. He can't really refuse to pursue a line of enquiry simply because it is upsetting to a lot of people.

No, I very much doubt it. You don't get a payout when someone dies anyway...generally only if someone is badly injured.
