Soundwaves Festival 2011 - featuring big-name international acts such as Iron Maiden, Slash, Queens Of The Stone Age and 30 Seconds to Mars - was to bring 40,000 fans to Sydney's Eastern Creek on February 27.
But just three years after it was moved from the inner city after traffic concerns, Soundwaves is again under threat.
The festival's future was thrown into doubt after an RTA report revealed traffic control was so poor at this year's event, pedestrians walked down M4 off-ramps.
The report also said that between 9am and 2pm on the day of the last concert, traffic was queued into high-speed sections on three major highways, "creating the environment for rear-end collisions".
Blacktown Council has vowed to reject the 2011 event unless the traffic plan is fixed, despite nearly all the $160 tickets being sold.
A council report said there were legal threats "should the event proceed based on a deficient traffic management plan which results in accidents or death".
"The event is not currently supported due to the inadequacies of the draft traffic plan," the report said.
Eastern Creek International Raceway general manager Geoff Arnold said organisers were doing all they could to make authorities happy, including putting on 80 chartered buses, off-site parking, and a free shuttle from Blacktown railway station.
"It is imperative that we get this working effectively," Mr Arnold said.
Soundwaves promoter AJ Maddah said 80 private buses should halve the number of cars on the road.
"Part of the problem is there is no public transport. We have buses ... to ease the travel burden on those fans," he said.
