The club has just rung me, they cannot find where they have received anything, and I have double checked my statements and the cheque has not been cashed, but the young lady is going to go through again just incase it has been put aside and let me know, if not they will send me another form and will process immediately.

Same thing happened to me mate.
I got the new forms last week thanks Hellsy and sent them away again yesterday.
